At Dayton MMA , we strive to provide our members with the best quality experience for their martial arts journey. We understand that circumstances can change, leading you to modify or cancel your membership status. This form outlines the terms and conditions related to the cancellation of your academy membership.
Cancellation Process
- Notification: To initiate a cancellation, members must submit a written notice to Dayton MMA Academy at least [30] days before the intended cancellation date. This can be done via email to memberships@daytonmma.com. Subject line “Cancel Membership” or “Change Membership”
- Form Completion (Form is below): Members are required to complete a Cancellation Form, available at the Academys website online at www.daytonmma.com/membershipchanges
- Final Payment: Any outstanding dues must be settled before the cancellation can be processed. If your cancellation request is not within the [30]-day notice period, one final payment may be charged
** Active members will continue to be charged until canceled. Memberships will not be automatically canceled due to lack of attendance **
Medical or Relocation Cancellations
Exceptions to our cancellation policy are considered on a case-by-case basis for medical reasons etc.
- Medical Cancellations: If you are unable to continue at the Academy due to medical reasons, be sure to communicate with us – and we will do our best to work with you. Often times we have seen injuries etc and can help with a training plan to allow you to keep training lightly working through the injury – we are not doctors and do not take any responsibility for your actions
- Relocation Cancellations: If you relocate more than 30 miles from Dayton MMA Academy – we totally understand, just use the guidelines above and we will get your cancelled – wish you well and send you on with our best wishes
Refunds
The Dayton MMA Academy does not offer refunds for membership cancellations except under exceptional circumstances, which will be considered on a case-by-case basis.
Policy Changes
Dayton MMA Academy reserves the right to modify this cancellation policy. All members will be notified of any changes [30] days prior to their implementation. If you contact us AFTER the 1st of the month, you will receive and owe the following months invoice in FULL. We do not believe in contracts, we DO NOT do signup fees, testing fees, pay months in advanced etc.
